Aerodrome Safety Matters Reporting

PLEASE READ AND AGREE THE FOLLOWING

The establishment of the Aerodrome Safety Matters Report System is intended to identify potential hazards to flight safety. Through the collection, investigation and analysis of various information relating to flight safety risks, necessary measures are taken with a view to preventing the occurrence of flight safety incidents.

Under §15.1.2 of the Personal Information Protection Act “…the government agency should not collect or process personal information unless there is a specific purpose and should comply with one of the following conditions:… 2. consent has been given by the Party”. For the purpose of identifying potential hazards to flight safety, your personal information will be required. The Aviation Safety Voluntary Reporting System is a non-punitive, confidential reporting system. Your personal information will be removed after it is processed.

You will be contacted if we find it necessary to obtain further information with respect to your report, or when you tick the box in your report that the reply is required.
